A new retinol goes on three nights running, and soon the face is tight, pink and flaking at the sides of the nose. Dermatologists' advice on avoiding that is consistent: start slowly, use less than you think, and give the skin time to adjust.

Below is a week-by-week starting schedule, what to pair with retinol, what to keep apart, and when to stop.

What the guidance says

The AAD, the US body for dermatologists, recommends a gentle formula at the lowest strength you can find, used every other night at first and built up slowly. Cleveland Clinic agrees: every other day for a couple of weeks, then increase gradually.

The AAD also notes that retinol products need not state their percentage. I think a higher number on the label is more often a marketing choice than a better product.

Why going slowly matters

Dryness, itching, redness and peeling are expected early on, and should ease as skin adjusts. On darker skin, the AAD warns, irritation can leave dark marks. A slow start and moisturiser help. Fine lines usually take several months to change, so rushing gains nothing.

A week-by-week starting schedule

WeeksHow oftenMove on if
BeforePatch test, a few daysNo marked redness or itch
1–2Every other nightSkin feels comfortable
3–4Two nights on, one offStill calm
5 onwardsNightly, if toleratedOtherwise stay put

A reasonable pace built on that guidance, not a fixed rule. If a step stings, go back one.

How to apply it

  • Night only. Retinoids raise sun sensitivity.
  • A pea-sized amount for the whole face, in a thin layer.
  • Dry skin. Wash gently, pat dry, then wait; Cleveland Clinic suggests 30 minutes.
  • Keep clear of eyes, nostrils and mouth.
  • Finish with a plain moisturiser that will not clog pores.

Hidden tip: one layer of moisturiser may be enough

The popular "retinol sandwich" puts moisturiser before and after. A small laboratory study, presented to the AAD and funded by a moisturiser maker, found that one layer, either before or after, kept retinoid activity comparable to retinoid alone; the full sandwich cut it roughly threefold. It used one donor's skin samples, so treat it as a pointer: buffer once, not twice.

Pair, separate, avoid

  • Pair: moisturiser at night; sunscreen of at least SPF 30 every morning.
  • Separate: vitamin C in the morning, retinol at night. With glycolic or salicylic acid, alternate days or split morning and night.
  • Avoid: scrubs, sunburned or broken skin, and a second retinol product at the same time.

Hidden tip: pause before a facial wax

The AAD advises stopping retinol and prescription retinoids two to five days before waxing the face, so the wax does not lift skin with the hair. Brow and lip waxing count too.

When not to start

The AAD advises against retinoids during pregnancy, and suggests seeing a dermatologist first if there is significant redness that may be rosacea. DermNet notes they can make eczema worse. At the counter, we often meet people who changed three products at once and cannot tell which caused the reaction. Introduce products singly.

Bottom line: the slow start is what lets you keep using retinol.

Retinol starter checklist card: patch test, every other night for two weeks, a pea-sized amount at night on dry skin, one layer of moisturiser, SPF in the morning
